The R-4 For those of you planning to home school independently, you may or may not choose to file an R-4 form with the state. This is an affidavit that you are a private school. It is not a request to have a private school. It is a notification that you are a private school. There is very little annual paperwork to maintain for families filing R-4’s. You need to keep an attendance sheet. You also need a Teacher’s resume and a course of study for each child. Finally, you need a health form or immunization record for each child. This year the R-4/PSA is going to be processed only by the state, instead of the individual counties. Those with computer access will be offered the opportunity to submit the form electronically via the internet. The R-4/PSA is also available from the state as a paper copy to be mailed in. CHN's legal committee has carefully evaluated the new procedure, and their suggestion is that if you plan to file an R-4/PSA this year, the most secure way to do so might be by mail only. They have identified some security risks to e-filing, including that the state is not using a secure server, and your information could readily be picked up by outside hackers. And, for those of you with 5 or fewer students, we know that some have worried that e-filing will make it easier for the state to track you. Although we don't have proof that this would happen, we think it's going to be just as easy to mail your affidavit and not worry about it.
